Child’s Birth Certificate (1 Copy) OPW requires a copy of your child’s birth certificate.

FINAL Report Card (1 Copy) All participants need to turn-in a copy of their FINAL report card that they will receive in June. Final report cards are due by, 6/30/2026. If on the report card there is a place for a teacher’s signature and grade for next year, this MUST be completed. If report cards are received without a teacher’s signature and grade, your child will not be able to check out gear or be able to practice.
