2024 Physical All Physicals must use the approved Pop Warner Form. (Kaiser check out forms will not be accepted, Kaiser does create their own Pop Warner form)

Child’s Birth Certificate (1 Copy) OPW requires a copy of your child’s birth certificate.
FINAL Report Card (1 Copy) All participants need to turn-in a copy of their FINAL report card that they will receive in June. Final report cards are due by, TBD. If on the report card there is a place for a teacher’s signature and grade for next year, this MUST be completed. If report cards are received without a teacher’s signature and grade, your child will not be able to check out gear or be able to practice.
